Payment
Payments, in accordance with the schedule made available, will be made on a pay as you go basis and payments can be made either by cash or cheque with a supporting bankers card and a receipt will be issued.

Block payments can be made in advance of sessions commencing.

Initial Assessment
All prospective clients will be expected to undertake an initial assessment before further therapy can take place.

Notes and Documentation
During the initial assessment appointment an intake form shall be completed along with an outline treatment plan. A copy of this will be made available to you, the client. Following each further appointment contact session notes will be made.

All personal documents and notes will remain confidential, unless for some legal reason I am required to release them to an appropriate authority, in this case I will inform you of the request.
They will be kept under lock and key in accordance with the Data Protection Act 1998, and destroyed at the appropriate time as set down by the Act.

Appointments and Cancellations
All appointments are booked in good faith, and if you have to re-arrange an appointment this must be done with at least 24 hours clear notice, if you have to cancel or re-arrange any one appointment more than twice, without good reason, the treatment plan will be cancelled.

Future appointments can be made either in advance, in which case you are more likely to get the same day / time, or from one session to the next which may mean that your regular day / time slot may not available.

If, in the opinion, of the therapist you arrive for a session whilst under the influence of alcohol or drugs, or you become abusive during a session then the session will be cancelled immediately and any money paid or owing will become due in full.

You are welcome to have someone with you for the first part of the Initial Assessment but generally speaking having a third person present during therapy sessions is not advisable and is discouraged, unless the client is a minor. You can have someone drop you off or pick you up at the designated times but as we do not have waiting room facilities they are advised to either wait in their car or go into the local village where they can park up and have a coffee, then come back to pick you up.

Termination of Therapy
Therapy can be formerly terminated by either party, at any point in time with at least 24 hours notice and this must be followed up in writing to formalise the situation. If termination is by the therapist, then you will be told why it has been terminated and what, if any, alternative treatments may be available to you in the local area. Furthermore, if treatment is formally terminated by either party then any monies pre-paid for future appointments will be refunded. If the termination of treatment is made by you, the client, and not formalised then any monies pre-paid will be forfeit.

Registration and Insurances
I am accredited, registered and licensed to practice as a hypnotherapist with the National Council for Hypnotherapy; I am also qualified and registered to practice as an Occupational Therapist in Mental Health with the Health Professions Council and I hold all public and professional Liability Insurances with Balens Insurance Brokers. My certificates of authentication are available for viewing upon request.

Finally
Whilst it might be difficult for you at times I have high hopes and expectations that your experience of therapy is eventually going to be a positive one and if you are happy with the service you receive please tell your family and friends, and if you are not happy with the service please tell me, so that I may have an opportunity to address the problem.
